आवेदन के साथ निम्नलिखित आवश्यक दस्तावेज जमा करने होंगे।

आधार कार्ड
जन्म प्रमाण पत्र
आयवेदक का बी०पी0एल० की प्रकाशित सूधी अथवा आय प्रमाण पत्र (यदि बी0पी0एल० सूधी में नाम न हो)।
अनाथ चच्चे की स्धिति में माता एवं पिता का सक्षग प्राधिकार द्वारा निर्गत मृत्यु प्रमाण-पत्र।
विधवा माता की संतान होने की स्थिति में पिता का मृत्यु प्रमाण-पत्र।
तालाकशुदा/परित्यक्ता की संतान होने की स्थिति में राक्षम प्राधिकार द्वारा निर्गत प्रमाण-पत्र।
जिसके माता या पिता गंभीर बीमारी (कैंसर, एड्स आदि) से पीड़ित होने की स्थिति में सदर अस्पताल द्वारा निर्गत चिकित्सा प्रमाण पत्र।
जिसके माता/पिता (कमाउ सदस्य) दुर्घटना/मानसिक बीमारी से पीड़ित होने की स्थिति में 60 प्रतिशत या उससे अधिक का विकलांगता प्रमाण पत्र।
कैदी पिता अथवा माता की संतान होने की स्थिति में न्यायालय द्वारा निर्गत प्रमाणित आदेश के साथ अन्य अभिभावक (पिता अथया माता) येरोजगार हैं, से संबंधित घोपणा पत्र संलग्न करना अनिवार्य है।
